Policy Information

This form tells you all about our policies here at New Tampa Pet Resort. You can learn everything from our hours to our policies and payments accepted for our many services. Inform yourself on all of the basic things you need to know about our resort before you and your pet stop by.

Guest Information Form

This form gives us a head start in getting to know you. We form the best relationships with our clients by getting to know them on a personal level. The more we know about you and your pets, the better we can do our job. Most clients become like family hear so we can anticipate problems before they are brought up. It creates a good working relationship so we can ensure that your pet can come enjoy our services for a long time to come!

Additional Pet Form

Here we have an additional form that further allows us to get to know you and your pet. This form asks for more personal and medical information so we can make sure we are on top of everything.

Health Care Commitment Form

We are very pleased to offer our Health Care Commitment (HCC) Program at New Tampa Pet Resort. This form includes our Policies and HCC information, and requires an electronic signature from all guests. The HCC program helps cover veterinary care for the rare instance that your pet is injured or becomes ill during their visit to our resort. The form goes into all details including what is covered, what is not covered, and reimbursement amount and eligibility. Please read carefully and don’t hesitate to reach out if you have questions.
